This distribution leaves neighbor islands with limited capacity to conduct local studies on asthma prevalence or to adapt interventions for Native Hawaiian and Pacific Islander populations, who experience asthma-related emergency visits at rates 40% higher than state averages.
Native Hawaiian communities on the Big Island and Maui face additional barriers because existing education materials do not incorporate traditional concepts of health such as ola or the use of native plant-based remedies alongside standard pharmacotherapy. Researchers attempting to address these gaps must account for the state's geographic isolation, which increases the cost and time required to recruit participants and ship laboratory samples to mainland collaborators. The resulting shortage of culturally validated protocols limits the ability of early-career investigators to generate locally relevant pilot data.
