Hawaii's inter-island transportation constraints limit consistent delivery of cardiovascular education programs. With eight main islands and commercial flight schedules that concentrate service between Honolulu and the Neighbor Islands, nonprofits face recurring gaps in reaching residents on Kauai, Maui, Molokai, Lanai, and Hawaii Island during weather disruptions or peak tourism periods.

Native Hawaiian and Pacific Islander communities concentrated on the Neighbor Islands experience the highest rates of cardiovascular risk factors, including hypertension prevalence 18 percent above the state average. Filipino and Japanese populations on Oahu also show elevated neuromuscular condition incidence linked to occupational patterns in agriculture and construction. These groups encounter additional barriers when programs remain anchored at Oahu facilities.

Workforce distribution compounds the issue. The state records 2.4 cardiologists per 100,000 residents outside Honolulu County compared with 9.1 per 100,000 in Honolulu County. Medical education programs at the John A. Burns School of Medicine produce limited numbers of specialists willing to maintain regular Neighbor Island clinics.

The proposed series of workshops addresses these constraints by rotating sessions across county health centers and community colleges on each island. Local physicians and nurses already credentialed in their home counties deliver content, eliminating repeated inter-island travel for a single presenter. Sessions incorporate place-based examples such as poi-based sodium reduction demonstrations and fishing community physical activity modules.

Funding supports staggered scheduling that aligns with existing primary care visit cycles at federally qualified health centers. Each workshop includes pre-session chart reviews so participants receive individualized risk assessments rather than generic presentations. Data collection occurs through the state's existing Hawaii Health Information Exchange, allowing follow-up tracking without new infrastructure.

Unlike California or Washington programs that can rely on contiguous road networks, Hawaii applications must demonstrate explicit coordination with the Hawaii Department of Transportation's airport and ferry schedules plus backup virtual options for Molokai and Lanai when commercial flights are canceled.
