Hawaii's Research Workforce Constraints in Genetic Sarcoma Studies
Hawaii maintains a limited pool of researchers trained in population genetics and oncology. The state has approximately 1.4 million residents spread across six major islands, yet only two institutionsthe University of Hawaii Cancer Center and the John A. Burns School of Medicinehouse dedicated sarcoma research teams. This concentration leaves neighbor islands without local expertise for tissue collection or longitudinal follow-up.
Native Hawaiian, Filipino, and Japanese populations each carry distinct allele frequencies that differ measurably from continental U.S. cohorts. Researchers must therefore collect and genotype samples locally rather than rely on external biorepositories. Current workforce data show just 11 board-certified medical geneticists practicing statewide, with six based in Honolulu County. This distribution creates delays in variant interpretation and counseling for patients on Kauai or the Big Island.
The funding targets salary support for two new research-track faculty positions per year at the Cancer Center and provides stipends for three postdoctoral fellows focused on admixture mapping. It also covers travel costs for sample transport via inter-island courier networks that operate under strict temperature-control protocols. Eligible applicants must demonstrate prior publication using Hawaiian cohort data or documented partnerships with Hawaii Pacific Health and the VA Pacific Islands Health Care System.
Unlike applications from states with contiguous geography, proposals must include explicit plans for maintaining chain of custody across commercial air carriers and county health department permits. Reviewers will assess whether staffing models account for the 2,500-mile distance to the nearest mainland reference laboratory. Successful projects have previously shown that local hiring reduces sample attrition from 18 percent to under 4 percent compared with mainland coordination.
Workforce expansion also requires integration with existing clinical pipelines. The program therefore requires letters of support from the Hawaii Department of Health's Chronic Disease Prevention Branch and from at least one federally qualified health center on a neighbor island. These documents must detail how new staff will allocate time between research protocols and existing oncology clinics in Hilo and Kona. Quarterly reporting on filled positions and retention rates will be mandatory for continued disbursement.
