This distribution restricts the ability of nonprofit institutions to host investigators pursuing high-risk projects on cancer mechanisms that incorporate local environmental exposures such as volcanic emissions. Workforce constraints further compound these gaps. The University of Hawaii Cancer Center reports 22 percent vacancy rates in molecular biology positions, and neighbor-island hospitals maintain no dedicated cancer biobanking staff. Researchers must therefore rely on Oahu-based core facilities, which schedule equipment time months in advance. These shortages directly affect projects requiring rapid sample processing for genetic or proteomic analysis. Broadband access remains uneven, with 38 percent of rural census tracts lacking speeds sufficient for real-time data transfer to national repositories. Nonprofit research organizations must demonstrate existing secure data pipelines before submitting proposals, as the funding mechanism does not provide infrastructure supplements. Applications from Hawaii must therefore document specific readiness measures. Eligible nonprofits must list confirmed access to CLIA-certified laboratories within the state or formal agreements with mainland partners for overflow sequencing capacity. Proposals also require letters confirming that at least one co-investigator holds an active Hawaii medical license to handle patient-derived samples collected under local institutional review board protocols. Unlike applications from states with integrated academic medical centers, Hawaii submissions undergo additional review for inter-island transport compliance with federal aviation and biosafety regulations.
