The state's five major islands operate under separate county health systems, yet training programs remain concentrated on Oahu, creating documented gaps where neighbor-island providers must travel 200-plus miles for certification updates.

Rural Hawaii counties report I/DD service caseloads per provider that are 2.4 times higher than Oahu averages, according to state health planning data. Native Hawaiian and Pacific Islander communities on the Big Island and Maui face additional layers because existing training curricula rarely incorporate cultural protocols around family decision-making and traditional healing practices.

Workforce constraints are compounded by inter-island transportation costs. Providers in Kauai and Molokai have no local universities offering I/DD-specific Medicaid billing or care coordination modules. Broadband limitations further restrict access to online modules hosted by mainland vendors, with 34 percent of rural healthcare facilities reporting speeds below federal telehealth thresholds.

This funding can address these capacity shortfalls by requiring training delivery through partnerships with University of Hawaii community college campuses on each island. Funds may cover development of culturally adapted modules on Medicare and Medicaid eligibility rules specific to Hawaii's QUEST Integration program. Equipment purchases for simulation labs at Hawaii Community College and Kauai Community College would allow hands-on practice without Oahu travel.

Readiness requirements include demonstrated ability to coordinate with the Hawaii Department of Health's Developmental Disabilities Division and to track participant completion rates by island. Applicants must submit letters from at least two neighbor-island provider networks confirming they can release staff for training blocks of five or more consecutive days.
